Regional One Health Community Health Worker Program Earns Prestigious Accreditation

Regional One Health is proud to announce its accreditation for its Community Health Worker (CHW) Program, reinforcing its commitment to improving patient outcomes and better access to care in the community.  This accreditation recognizes Regional One Health’s dedication to providing quality, person-centered care through trained CHWs who serve as vital links between medical teams and the diverse populations they support.

The accreditation, awarded by Tennessee Community Health Worker Association (TNCHWA), ensures that Regional One Health meets rigorous standards in CHW training, program effectiveness, and patient support. Community Health Workers play a crucial role in healthcare by helping patients navigate complex medical systems, addressing social determinants of health, and promoting preventive care.

The CHWs work with patients for up to six months and provide essential services to program participants including care coordination and health education to improve continuity of care and management of chronic conditions.  They also address social drivers of health such as assisting with access to transportation, food security, housing, and other barriers that often prevent patients from achieving good health.  CHWs often come from the communities they serve, leading to trust and improved engagement with program participants.

The accreditation process was a multi-month endeavor, including the review of Regional One Health’s adherence to standards including CHW training, support, evaluations, development, scope of work and more.  The accreditation process culminated in a site visit, which Regional One Health completed with no findings for improvement.

TennCare partners with TNCHWA on the accreditation initiative to ensure organizations adhere to the best practices essential for developing quality CHW programs.  TennCare leaders commended Regional One Health’s accreditation.
