Qsource Community Partnerships (QCP) is accepting applications for a new grant-funding program to support community projects that pursue improvement in health outcomes.
Established by Qsource, the grants will support health improvement projects conducted in Illinois, Indiana, Iowa, Kansas, Kentucky, Missouri, Nebraska, and Tennessee. The deadline to submit an initial Letter of Interest is August 21, 2018. Applicants must be a nonprofit 501(c) (3) organization, an accredited educational program/institution, or a governmental entity.
Qsource is a nonprofit, 501(c) (3) healthcare quality improvement and information technology consultancy headquartered in Tennessee since 1973.
According to the Notice of Funding Availability released by Qsource, the organization expects to provide funding assistance for a variety of community projects and programs through multiple grant awards of up to $20,000 each. Qsource places a priority on funding projects that are designed to drive impacts in at least one of the following priority areas:
• Address local community health needs.
• Serve as a catalyst for innovation.
• Improve one or more health outcomes.
